Under direction of the Clinical Supervisor, you will provide case management services through direct contact with clients, families and essential others in their natural environment. You will serve as client advocate, acting in their best interest at all times, and attend treatment plan development and review meetings. You will also coordinate and monitor appropriateness of all services; examine and recommend new programs; provide assistance in crisis situations to stabilize clients.
How you’ll make a difference:
You will provide emergency services such as aiding in the coordination of services to stabilize client in the least restrictive setting.
You will record client progress, monitor relapse plan, maintain necessary and pertinent records and files.
You will develop/maintain consultative relationships with other agencies and programs in order to cultivate working relationships. You will also complete referrals and provide linkage to other agency programs, external service providers, and other community resources, as deemed clinically appropriate
You will ensure adequate client care, provide consultative service and/or promote the coordination and development of mental health and/or substance abuse services
You will maintain strengths-based approach while working with clients and families to assist in the retention of dignity and the development of positive intrinsic qualities in the client

Who are you?
You are a professional with any combination of education, training and work experience which indicates possession of the skills, knowledge and abilities needed to perform the job requirements listed. An example of an acceptable qualification for this position is: Associates in Social Work or related field with relevant experience related to the job requirements . You will have the ability to obtain CPR/First Aid Certification; clear Criminal Background Check, an Ohio driver’s license, maintain auto insurance, and fingerprinting will be required. You must meet the minimum required level of auto insurance per carrier guidelines, as the position is required to transport clients in your personal vehicle as needed.
