Job Description

The purpose of the Office Nurse-Pal Careposition is to assist the clinical team and clinical manager with triaging phone calls and transcription of orders. This position is also to be a clinical resource for the Bridge Team in accordance with all current applicable federal, state and local standards, guidelines and regulations.

Duty 1: Demonstrates the knowledge and skill to administratively assist the Home Health Team or Hospice Team in providing consistent quality patient care to those being referred to or receiving Home Health or Hospice services.

Duty 2: Demonstrates knowledge and skill necessary to provide disease/diagnosis age specific care to the patients served with Bridge Palliative Care.

Duty 3: Triaging phone calls and provide guidance to patient or patient’s caregiver per the plan of care. Makes prn nursing visits within scope of license.

Duty 4: Works with Clinical Manager and other associates in meeting with referral sources as needed and as appropriate regarding Bridge services.

Duty 5: Completes assigned tasks within Electronic Medical Records software as assigned to facilitate even workflow and complete patient medical record per Manager.

Duty 6: Manages on-call and helper assignments responsible to include keeping management informed of any unusual or special incident occurrences.

Duty 7: Complies with organizational policies, procedures, and practices to ensure quality job performance and Medicare/Medicaid compliance.

Duty 8: Documentation is consistent with or exceeds Bridge Palliative Care guidelines

Duty 10: Able to communicate criteria for admission to Home Health or Hospice.

Job Requirements


* Possesses a LPN licensure for the state of Ohio.
* Worked a minimum of 2-3 years of general nursing experience and 1-2 years of Home Health or Hospice nursing experience required.
* Knowledge and commitment of the philosophy, mission, and goals of the agency and standards and regulations governing it.
* A valid driver’s license is required (if you do not have a valid Ohio driver’s license you must obtain one within one year of your residency in the state) and you must also meet BVHS’s company fleet policy requirements
* Positive service-oriented interpersonal and communication skills required.
* Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status. Must be able to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs and to provide the care needed as described in the area’s policies and procedures
* This position is classified “at risk” for possible occupational exposure to blood borne pathogens (HBV, HIV, etc.
