Job Description

PURPOSE OF THIS POSITION

The purpose of this position is to assist with coordination of home health and hospice clinical team scheduling, assisting Intake with referral entry, and managing documents via our document management system.

DELEGATION OF AUTHORITY

You are delegated the authority, responsibility, and accountability necessary to carry out your assigned duties.

JOB DUTIES/RESPONSIBILITIES

Duty 1: Works collaboratively with intake department to ensure timely initiation of care.

Duty 2: Coordinates with members of the Home Health and Hospice Team and to optimize productivity.

Duty 3: Works closely with the Office Nurse for maintaining Aide schedule s to ensure patient scheduling needs are met.

Duty 4: Completing electronic medical record software tasks in support of clinicians.

Duty 5: Works collaboratively with Authorization Specialist to ensure patient scheduling needs are met.

Duty 6: Answers incoming phone calls and walk – in inquiries specific to assigned department and facilitates problem resolution.

Duty 7: Monitors and reviews documents in document management system and forwards to appropriate person.

Duty 8: Serves as back up to the F2F Specialist.

Duty 9: Displays Service Excellence as evidenced by practicing the Mission, Vision, and Values of the organization to promote customer satisfaction.

Duty 10: Complies with organization policies, procedures, and practices to ensure quality job performance and Medicare/Medicaid compliance.

Duty 11: Completes other duties as assigned.

Job Requirements

REQUIREMENTS

* High School Diploma.
* Experience in Microsoft Office applications .
* Excellent organizational and customer service skills .
* Great verbal and written communication skills .
* Excellent critical thinking skills.

PREFERENCES

* Medical background with medical terminology.

PHYSICAL DEMANDS

This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel .
